Modified Interpretations of the Supine Roll Test in Horizontal Canal BPPV

New research on the Supine Roll Test (SRT) in horizontal canal BPPV reveals how initial debris position and test sequence affect nystagmus patterns and diagnosis. Thanks to Bhandari et al. (2022). (Front Neurol.) for this 3D simulation study which found that starting the SRT from the right or left side generates different nystagmus types—bilateral direction-changing, bilateral direction-fixed, or unilateral—depending on debris location. These findings suggest a standardised test protocol is needed for more accurate diagnoses.
